Description of REACH
The REACH Program of Evergreen Treatment Services works with individuals experiencing homelessness and behavioral health conditions to help them achieve stability and improved quality of life. Through outreach, engagement, and ongoing relationships REACH focuses on reducing harm and supporting healing. REACH bridges gaps and helps individuals access essential resources such as housing, health care, entitlements and treatment. The REACH Team is passionate about creating a hospitable and welcoming environment for all people while providing quality services on an individually tailored basis to their clientele. REACH Neighborhood-Based Outreach Program
REACH has a contract with the City of Seattle to provide neighborhood-based outreach and care coordination services to people living homeless in the Capitol Hill, First Hill, and Chinatown/International District areas. There are three full-time positions in this collaboration between REACH and the neighborhood Business Improvement Associations (Capitol Hill, First Hill, Chinatown/International District). These three positions will coordinate with each other and with other REACH outreach workers to conduct street outreach in alignment with best practices for safety and client engagement. There will be a designated REACH point of contact for each of the three BIA’s, though outreach may often be conducted in pairs in all three areas. These positions will engage homeless individuals in these neighborhoods and help facilitate linkages to resources and housing. Each fulltime outreach position will provide services to homeless individuals for the following purposes:
- Improve community health and safety by working to resolve the underlying needs of people living on the streets;
- Focus on the highest impact people and most problematic locations; REACH
- Provide information and education to the community about the state of homelessness and opportunities to care for individuals on the street in their community.
The REACH Outreach Care Coordinator(s) will provide a range of services to people living outside, including Coordinated Entry Assessments; transportation assistance to home community; survival services; connections to shelters/sanctioned encampments; and/or medical/mental health/substance use disorder referral and treatment.
Job Summary
This position entails going out into the community to contact, develop rapport, and engage with individuals who are living outside. The purpose of this outreach is to offer information, referral and direct supportive services to help these individuals access needed community resources. Services are to be provided from a client centered, harm reduction approach, utilizing motivational interviewing skills. The Outreach Care Coordinator may also connect with community members and business owners in the area to help inform them about current conditions for homeless individuals in the community and help strategize positive solutions to homelessness in their neighborhood. The nature of this position is extremely independent, requiring excellent interpersonal relationship and communication skills, common sense regarding personal safety, creative thinking, multi-tasking and conflict resolution skills
Job Function
- Provide effective services to individuals living outside, through: o Initiating contact with individuals living outside and explaining available services.
- Through repeated contacts, developing relationships with individuals in order to assess their service needs and responding with appropriate referrals and linkages to resources.
- Helping individuals to reduce barriers to obtaining housing and providing direct support to access community resources such as personal identification, shelter, housing, entitlements, medical care, behavioral health treatment, etc.
- Referring eligible individuals to more intensive case management programs when indicated.
- Developing and maintaining productive relationships with all community partners through timely and effective communication and participation in all required meetings and activities.
- Developing and maintaining a working relationship with other Outreach programs to facilitate effective collaborative efforts and prevent needless duplication of services.
- Developing and maintaining effective relationships with shelters and other programs that provide direct services to the target population and coordinating client access to these resources.
- Providing Outreach Care Coordination services consistent with ETS/REACH policies and procedures.
- Documenting all activities and tracking all expenses accurately and in a timely manner.
- Advocating for individuals living outside with a wide variety of other service providers.
- Identifying gaps and barriers in available community resources and advocating for systemic changes to improve quality and coordination of community services.
- Attending REACH and other required meetings. o Documenting activities according to agency, contract and state requirements.
- Representing the REACH Outreach program to community stakeholders using effective communication and strategic partnerships
- to best leverage the REACH program’s strengths and contribute to the success of the program.
- Local travel on behalf of the agency is a job requirement.

Education
- High school diploma or equivalent required, undergraduate degree with coursework in the social service field preferred.
Experience
- minimum of two years’ experience in work related to social work or outreach programs.
- Demonstrated ability to respectfully engage with individuals experiencing homelessness and behavioral health conditions essential.
- Experience providing services from a harm reduction perspective highly desirable.
Requirements
- Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.
- Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
- Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.
- Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.
- Must be able to pass a Washington State Patrol criminal background check.

- The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
