Job Overview for Oasis RN
We are looking for an experienced Oasis RN to join our patient nursing team and provide Start of Care services to patients in their primary residence. The right candidate is a patient, compassionate professional who encourages and inspires their patients to work toward greater independence and recovery.
Qualifications
Responsibilities for Oasis RN
- Perform comprehensive Oasis assessments to establish a care plan for the patient.
- Develop and document the plan of care in the applicable EMR system.
- Facilitate communication between the patient and the applicable clinical team.
- Perform other Registered Nurse visits as needed.
- Assess and chart observations of the patient’s condition at each visit.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Dress or redress wounds and assess the healing progress.
- Provide education to patients and families on proper home health care procedures and strategies.
- Recommend tools or devices that may improve the quality of life for the patient.
- Oversee other workers such as LPNs, nursing aides, and home care aides.
Qualifications for Oasis RN
- Proven ability of performing OASIS assessments in the home care environment.
- Proven record of providing patients with nursing care.
- Ability to exercise independent and sound judgement.
- Strong decision-making skills and well-developed interpersonal skills.
- Effective organizational and time management skills along with strong teamwork skills.
- Experience with a wide range of patient age groups and disorders.
Experience and Education for Oasis RN
- Prior experience in performing OASIS assessments.
- Experience in providing nursing care to patients in a residential setting or similar environment.
- Current Registered Nurse license for the state of operation.
