SUMMARY


Will help consumers move through the complexities of the behavioral healthcare system ensuring connected care during the application/admissions process for FNA’s clinical treatment programs,     preventing applicants from falling through the cracks and reducing the risk of relapse and return to intensive services. Will be responsible for providing case management services for consumers in the application process and monitor the applicant through disposition, treatment admissions, program transfers and discharge.  


JOB DUTIES

  • Provides and conducts case management services to include scheduling and data (billing) entry, as well as regular tracking of internal and external referrals for FNA’s clinical treatment programs. Collects necessary documentation to prioritize applicants and prepare disposition documentation to determine treatment approval or referral.
  • Provides ASAM Assessments to determine level of care recommendations and other behavioral health screening tools to identify consumers with moderate to high-risk mental health symptoms and substance abusing behaviors ensuring care coordination and advocacy for options and services to meet an applicant’s individual and family’s comprehensive needs and supports.
  • Maintains regular contact with treatment applicants through face to face, virtual, telephonic and written communications utilizing consumer centered and trauma informed care approaches. 
  • Creates and monitors pre-treatment change plans for waitlisted consumers. 
  • Monitors consumers through the treatment process, ensuring accurate treatment bed/slot reservations. Follows-up with consumers who drop out of treatment, offering reassessment of needs providing case management to appropriate referrals.
  • Enters accurate and timely data into the Electronic Health Record (EHR)
  • Develops and fosters relationships within the community, stakeholders, and other entities.
  • Builds and maintains an extensive database of community resources to match consumers with eligible resources.
  • Coordinates directly with treatment providers to track and ensure follow-up services are being provided, as well as the progress of referrals. 
  • Responsible for providing culturally relevant and trauma informed service. 
  • Promptly responds to incidents and other events as necessary.
  • Acts as a positive representation of FNA to the community.
  • Performs other job-related duties as assigned.

 

NECESSARY KNOWLEDGE, SKILLS, AND ABILITY

  • Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders
  • Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles
  • Skill in managing multiple, concurrent priorities and meeting deadlines
  • Skill in computer usage
  • Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers
  • Ability to work independently as well as with teams
  • Ability to report to work in a timely manner
  • Ability to write professionally and communicate effectively, and respectfully
  • Ability to apply customer service concepts and practices

 

MINIMUM QUALIFICATIONS (Education & Experience)

  • Bachelor’s degree in behavioral health or related field
  • Minimum two (2) years’ experience in case management
  • Minimum one (1) years’ experience working with Alaska Native/American Indian population and experience providing culturally specific services

 

PREFERRED QUALIFICATIONS (Education & Experience)

  • Experience serving Fairbanks and/or FNSB census area
  • Experience providing direct service to target population
  • Experience in behavioral health

 

WORKING ENVIRONMENT

This position corresponds to medium work, involving physical exertion of 20-50 lbs. frequently; such exertion involves the lifting and/or moving of young children in the program, and/or goods and materials within the facility.  There is frequent walking, standing, climbing, stooping, bending, and squatting/crouching.  The majority of work is performed in a classroom setting with a wide variety of people with differing functions, personalities, and abilities.  The office is a non-smoking facility with outside smoking areas.  May involve local travel between various program components of FNA and home visits where exposure to extreme weather conditions are likely.  Works in an environment where protection of client confidentiality is essential.  Position requires cleaning the class area every day, including putting supplies in proper space, cleaning and sanitizing surfaces with bleach solution to disinfect.   Personal Protective Equipment will be provided where/when needed. 

 

 

SPECIFICATIONS

Supervision Received:

Position receives direct supervision from the Clinical Supervisor; however, incumbent is expected to effectively perform day-to-day activity as denoted within job description.  Work will be monitored through weekly meetings, monthly reports, and regular communication with the Clinical Supervisor.  General guidelines for operations are based on program requirements and the general policies and procedures of FNA.

 

Supervision Exercised:

No supervision requirements for this position.

 

Position Impact:

Impact at the program level is high.  Program’s level of compliance with program standards is directly affected by the effective performance of duties, especially as they relate to providing program services.

Contacts may include, and not limited to, volunteers, organizational staff, consumers, and outside agencies.

This position will (or may) require you to frequently (or incidentally) drive your personal vehicle for FNA business.  Per FNA’s automotive insurance policy, in order to drive a FNA-owned vehicle, an individual must be at least 21 years of age.  In order to drive a FNA-owned vehicle with consumers in the vehicle, an individual must be at least 25 years of age.  No employee may transport a FNA consumer in a personal vehicle.

 

Continued employment status will be contingent upon satisfactory completion of a criminal background investigation and the pre-employment drug testing process.  Where required, continued employment status (post job offer) will be contingent upon employee providing certification of annual tuberculosis (TB) screening and medical certification that employee does not, because of infectious or communicable disease, pose a direct threat or significant risk to the health and safety of program participants or others in the workplace.
