We at Fairview Health Services are looking for a dedicated Team Coordinator to join the hard working team at the Riverside Clinic in Minneapolis. This is a full time, 80 hours every two weeks, benefit eligible position located at the University of Minnesota - West Bank Campus. The Riverside location has a diverse patient population and is one of the smaller clinics our system. It is a close knit staff with providers who focus on staff education. M Health Fairview - Riverside Clinic currently has 6 providers, but we are scheduled to start 3 more in the next 6 months! This clinic is a growing and sees patients from infant to adult. Responsibilities
As an integral member of the primary care team, the team coordinator contributes to overall quality of care and patient experience through performing high-level functions to support the needs of the healthcare team and patients/support network. We have TC's partner with all members of the team to ensure patients needs are optimally addressed. They will deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC will use a variety of communication tools and systems in their work to support patients and their care team. Additionally, we ask the TC's to maintain complete, timely and accurate documentation for each patient. They will also continually look for opportunities we can improve processes and workflow.
- Performs telephone & MyChart message support & follow-up communication in a timely and professional fashion; uses communication systems for both routine & emergency transactions
- Schedules, reschedules & maintains patient appointments & supports clinic leadership in schedule management
- Partners collaboratively with the patients care team to address patient needs
- Find opportunities for improvement in workflow & participate in clinic and system meetings
Qualifications
Required Qualifications
· At least 1-year of customer service experience
· Proficient with technology (computer hardware and programs, smart devices such as phones or tablets)
Preferred Qualifications
Current nursing, medical, social work, psychology, or pharmacy student
· Successful completion of, Nursing Assistant, Emergency Medical Technician, Heath Unit Coordinator program.
