Fairview Health Services is looking for a Team Coordinator based out of the Vadnais Heights Clinic. This position is part time, approved for 64 hours per two week pay period (FTE 0.8), and the position is benefit-eligible.
As an integral member of the primary care team, the team coordinator contributes to overall quality of care and patient experience through performing a variety of high-level functions to support the needs of the healthcare team and patients/families. The team coordinator partners with all members of the care team to ensure patient needs are effectively addressed in a timely manner. The team coordinator will provide direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The team coordinator uses a variety of communication tools and systems in their work to support patients and the care team. Additionally, the team coordinator will maintain complete, timely and accurate documentation for each patient. The team coordinator will also continually look for opportunities to improve processes and workflow.

Responsibilities
- Performs telephone and MyChart message support and follow up, directs communication in a timely and professional fashion
- Effectively uses communication systems for both routine and emergency transactions
- Schedules, reschedules and maintains patient appointments
- Partners collaboratively with the care team to address patient needs
- Schedules and/or facilitates scheduling of diagnostic tests, clinical appointments and consultations at specialty clinics on a limited basis
- Supports clinic leadership in schedule template management
- Assists with completion of tasks within specified work queues
- Performs multiple, simultaneous tasks in an environment faced with multiple interruptions
- Identify opportunities for improvement in workflow and participate in clinic and system meetings
Qualifications
Required Qualifications
- Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or minimum of 1 year comparable experience in a healthcare setting
Preferred Qualifications
- 2 or more years’ experience in a medical setting
Desired Skills
- Knowledge of medical terminology and abbreviations preferred
Wage: $18.13 per hour (depending on experience)
