Responsibilities Job Description
This Visit Facilitator is responsible for supporting in person clinic visits; including communicating with patients prior to their visit and helping guide patients through the care experience. Visit facilitators capture pertinent medical information from the patient & details the information into the medical record. This role also helps ensure the exam rooms are stocked, equipment & supplies are available, assists with tasks that support the effectiveness & efficiency of the team. The Visit Facilitator may also assist in scheduling & coordinating appointments, preparing schedules and other delegated duties as assigned.
- Prepare patients for their appointments including: communicates with patients in advance of their scheduled appointment to ensure medical records are collected, received & available for visit
- Assist in the rooming process by collecting & detailing pertinent information: reason for their visit, medication review, vitals collection with automated vitals machine & health questionnaires
- Supports the patient connection with the provider for the visit & serves as a resource ensuring a positive patient/provider experience.
- Completes visit wrap-up ensuring after-visit summary is provided & patients are aware of recommendations for follow up. This may include assisting with scheduling & coordinating follow-up appointments.
- Communicates efficiently with patients, providers & care team, demonstrating professional emotional intelligence in person & with virtual technologies
Qualifications
Required
- At least 1-year of customer service experience
- Proficient with technology (computer hardware and programs, smart devices such as phones or tablets)
Preferred
- Current nursing, medical, social work, psychology, or pharmacy student
- Successful completion of: NA, EMT, or HUC program
- Experience with Medical terminology, Epic Health Records, Telephonic customer experience and has previously worked in a health-care setting
