Positions at Bristol Lifestyle Recovery (Bristol VA) and Mended Women Lifestyle Recovery (Abingdon, VA); low-intensity residential communities for men and women challenged by substance abuse to discover the strength, skills and healing necessary to reclaim their lives and achieve their life purpose
Essential Job Functions Include:
- Provides medical education and care coordination and reenforces effective participation in their own medication management and overall healthy living factor. Notify residents and document if they are low on medication and help coordinate refills on an individualized basis. Provide assistance with primary care and related appointments. Works to equip Residents with the skills to continue effective medication management post-graduation and/or discharge.
- Responds to medical emergencies and other potential crisis situations (verbal or physical) by utilizing agency approved methods, i.e., CPR, Infection Control, Therapeutic Options, and other approved program procedures.
- Monitors medical needs of residents through initial meeting upon intake, initial and post review of medical-related SIR and weekly monitoring of diabetic and acute medical conditions
- Assists in review of medical appropriateness of admissions
- Coordinates with medical partners by corresponding, retrieving doctor’s orders and medication refill auditing and assurance.
- Train, retrain and develop new staff in Medication Administration and CPR/First Aid/AED as needed. Provide retraining and regular competency review for all staff participating in medication administration and management. Accordingly ensures documentation is completed and submitted into staff HR file
- In coordination with other licensed Nurses, receives and interprets external physician’s orders. Documents physician’s orders of medication to residents, and transfers information to MAR system.
- Administers medication and injections, and monitors compliance of staff who administer and record medication administration.
- Monitors resident compliance with physician’s orders and evaluations and coordinates such compliance with Clinical staff and primary prescriber.
- Provides oversight and monitoring of MARs, ensuring full compliance and effective rollovers.
- Ensures all Supportive Documentation related to medication administration, education, evaluations and management are entered into ECR and effectively communicated with Clinical provider(s).
- Receives individuals’ laboratory test results and observes individuals for changes in behaviors and symptoms
- Ensure medication counts are conducted after each med pass and/or at shift handoff and are properly recorded
- Ensure medication errors are being reported in a timely manner via Unusual Incident Report or Serious Incident Report as outlined in the Medication Administration Criteria (MAC)
- Provide regular oversite to ensure medication administration is following the MAC at assigned location and is documented into the Shift Handoff Report.
- Communicate and maintain regular contact with labs, pharmacies, doctors, etc. in assisting residents with coordination of medical care
- Participate in cross-departmental summit meetings and involvement in clinical activities as requested.
- May function as Acting Team Leader per shift request per Director of Operations or another Executive Team member.
- Provide education and nursing care to residents within the ASAM 3.1 context.
- Participates in resident treatment team meetings as requested, acting as a liaison between FHMC and the residents’ external medical provider
- Check email & Teams messages throughout the day and respond in a timely manner
- Count and document medications and ensure all Med Admin staff document and follow all procedures and requirement.
- Conduct UDS and cheek swabs as needed
- Provide Tuberculosis screenings as needed.
- Other job duties as assigned by immediate supervisor
