Responsibilities/Job Description:
- Gathers the data involved in total systems assessment, including physical, psychosocial and spiritual data, as well as ongoing assessments.
- Demonstrates awareness of age specific criteria in obtaining assessment data.
- Identifies patient needs and incorporates into an individualize plan of care.
- Assists the physician in coordinating a plan of care based on prioritized patient needs in collaboration with the patient, significant other and resource personnel.
- Identifies expected outcomes which are realistic, measurable and attainable.
Required:
- Must be willing to obtain certificate of "cancer basics" within 1 year of hire.
- Current Minnesota LPN Licensure.
- Current BLS or attends BLS class within six months of employment and maintains certification.
Preferred:
- Previous nursing experience.
- Oncology experience.
