MISSION STATEMENT: Fairview Recovery Services helps people with the disease of alcoholism, chemical dependency, and co-occurring conditions live independent, healthy, and productive lives by providing a continuum of individualized services and care.

DISTINGUISHING FEATURES OF THE LICENSED SOCIAL WORKER POSITION : The work involves responsibility for planning, providing and managing the clinical and program needs of the clients served by this project in the community residences and supportive living programs. The incumbent will have responsibility to provide case management (i.e. coordinating all services being provided to the clients), coordination of the service plan development and ongoing reviews, crisis intervention, supportive counseling, symptom monitoring and management, identifying appropriate referrals and communication with those referrals. In addition to formulating and implementing individualized plans designed to restore and enhance social functioning for individuals, the work involves designing and implementing programs for specific target populations. The position requires the incumbent to perform independent analysis and develop alternate counseling strategies and diversion techniques. The incumbent will be assigned highly complex cases that require active coordination with other agencies (Mental Health, Criminal Justice). The position requires the incumbent to provide training to FRS staff to increase their knowledge of the dually diagnosed population. Work is performed under the direct supervision of designated Program Director. The Social Worker performs related work as required.

TYPICAL WORK ACTIVITIES OF THE LICENSED SOCIAL WORKER POSITION: (Illustrative only)

· Provide psychotherapy to all residents who are in need of such services in a frequency sufficient to meet the assessed need;

· Provide individual, group (no more than 15 persons per group therapy session) and family counseling (which includes services to significant others) as appropriate to resident needs;

· Directly supervise groups facilitated by residents who have greater experience or seniority within the service. (This may also include supervision by other clinical staff members.);

· Provide Multi-family group counseling and psycho-education;

· Provide medical assessment of physical and mental health conditions and medical treatment to enable the resident to manage chronic health and mental health conditions including treatment of physical health conditions that are routine;

· Provide Psychiatric assessment and medication management of co-occurring psychiatric conditions which can be managed within the residential setting;

· Provide Psycho-social interventions which teach skills for coping with urges, craving, impulsive behavior and cognitive distortions in thinking, motivational interviewing techniques to engage the resident in treatment;

· Provide planned interactions with residents within the milieu intended to build social, emotional, and behavioral functioning including: increased empathy, successful social interactions, increase in self-efficacy, confidence, control over impulses, managing of urges and cravings to use and the skill in use of social supports available within the community;

· Assist with self-administration of medications, as needed

· Manage a small caseload, as needed;

· Identify appropriate clients for referral to the program;

· Coordinate the referral and discharge processes with the Admissions Coordinator;

· Review current psychotropic medication and written prescriptions, the personnel or agency responsible for providing each service, and the place at which these services will be provided;

· Review financial/economic arrangements, transportation arrangements, vocational/educational services and family involvement;

· Develop individualized service plans for clients;

· Analyze individual client’s problems;

· Coordinate the treatment target population within the agency on acting as Primary Therapist;

· Provide immediate supportive reinforcement to clients in crises intervention situations;

· Provide professional consultation services and formal reports to other FRS programs;

· Provide services to agencies such as reporting, analysis of behavior of clients, and recommendations for treatment in relation to chemical dependency;

· Perform activities related to the maintenance and individual follow-up to cases and client records;

· Prepare and maintains various reports and records as required;

Must be LCSW or LMSW.

A. Graduation from a regionally accredited or New York State registered college or university with a Master’s degree in social work and two (2) years combined direct care experience in a mental health and/or chemical dependency setting.

SPECIAL REQUIREMENTS FOR THE LICENSED SOCIAL WORKER POSITION: Must possess and maintain a valid driver's license with less than three (3) points and a good driving record

Paid Benefit Part-Time includes 2-4 hour Holidays plus 2 extra 4-hour Floating Holidays, 12 hours of Personal Time per year, 48 hours of Sick Time per year (which rolls over annually), 1 week of Vacation per year (which rolls over annually; maximum of 120 hours)

Full-Time positions are eligible for a benefit package that offers Health Insurance, Dental Insurance, Long Term Disability Insurance, AFLAC Plans, Flexible Spending Account (Cafeteria Plan), Life Insurance (after 1 year full time employment), 401k Retirement Plan with up to a 5% company match.

Paid benefit Full-Time include 6 Holidays plus 4 extra Floating Holidays, 24 hours of Personal Time per year, 96 hours of sick time per year (which rolls over annually), and 2 week’s Vacation per year with an increase of 8 hours annually earning up to 5 weeks of paid vacation.'
