What are the responsibilities and job description for the LVN Staff Nurse position at Falcon Wellbeing Care?
Falcon Wellbeing Care is a 6-bed Congregate Health Living Facility which provides inpatient care, including the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, social recreational, and much more. We are looking for a Licensed Vocational Nurse to care for our patients and facilitate their speedy recovery. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. The goal is to promote patient’s well-being by providing high quality nursing care.
Responsibilities
Mostly office experience to do phone triage, assist in completing 485s/Plan of Treatment (computerized program Matrixcare which codes and types treatment plans), review nursing and multidisciplinary notes, review Evals and Recerts for non Medicare Home Health Agency (no oasis)
- Quality Assurance/Quality Improvement activities including preforming quality management activities on charts
- Infection Control Log and write incident reports
- Telephone triage - new intake, order medical supplies, coordinate care of patients
- Review Field Staff Clinical Progress Notes
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Observe and interpret patient’s symptoms and communicate them to physicians
- Collaborate with physicians and nurses to devise individualized care plans for patients
- Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts
- Adjust and administer patient’s medication and provide treatments according to physician’s orders
- Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
- Provide instant medical care in emergencies
- Foster a supportive and compassionate environment to care for patients and their families
Qualifications
- Current BLS certification
- COVID-19 Vaccination (proof of vaccination required)
- Good understanding of nursing principles
- Good understanding of patient/family teaching principles
- Knowledge of computer practice management systems and other computerized systems (Microsoft Office programs, electronic health record systems)
- Good communication skills
- Current California LVN license
- Evidence of continuing education
- Current American Heart Association Basic Life Support Card
- Knowledge of medical terminology, infection control and safety
- Excellent knowledge of nursing care methods and procedures
- Excellent knowledge of emergency care
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills
- Previous experience preferred
