We are searching for a Case Manager to join our Supportive Housing for Families
team:

The SHF Case Manager will provide assessment, intensive home-based case management and housing assistance for families to ensure the safety and well-being of their children. We are a home based program working with families in their home and within the community to ensure they are able to maintain safe affordable housing while also building on their self-sufficiency skills. The program works closely with The Department of Children and Families as well as other community programs. Case Managers are responsible for a caseload of 12 to 15 cases located in lower Fairfield County, maintaining up to date and accurate electronic records and provide transportation for families as needed. This position is 35 hours/week, Monday – Friday. Nights and weekends as needed.


Requirements:

  • Bachelor’s degree in social work or other related human services field is required.
  • Two years’ experience in a similar position.
  • Strong written and verbal communication skills.
  • Family engagement and empowerment skills.
  • Valid Connecticut driver’s license and proof of insurance.
  • Must have reliable transportation to perform home visits.
  • Proficiency in making sustainable connections with families to local community agencies.
  • Knowledge of the service system for children and adults in Bridgeport, Norwalk and Stamford.
  • Meet contractual guidelines related to service plan development and case documentation.
  • Must have awareness of the cultural and socioeconomic characteristics of clients served.
  • Must be able to work with diverse populations and be willing to provide home based services.
  • Demonstrate maturity, professionalism and self-awareness, a non-judgmental attitude, the ability to deal with resistance and rejection, cultural competency and the ability to work independently and under challenging conditions.
  • Bilingual in general as Spanish or Creole candidates would be great.
