ESSENTIAL DUTIES AND RESPONSIBILITIES
This is a grant-funded position, which is set to expire at the end of June 2023. Although the grant is expected to end at that time, additional funding may be obtained which may allow your employment under the grant to be extended.
- Work closely with the School-Based Health Center staff and other Family First Health staff to coordinate and integrate health and well-being activities within the Hannah Penn K-8 School
- Serve as Family First Health point of contact and lead coordinator within the Hannah Penn K-8 School for schoolbased health activities
- Work with the team to coordinate and support activities related to Connections, a community collaborative of which Family First Health is the convener
- Collaborate with school and community partners to ensure effective delivery of school-based Health and Connections activities
- Support community outreach, engagement and communication with stakeholders and partners involved with Family First Health’s school-based health and Connections efforts, including events and activities within the school and in the community
- Lead the development of a youth health advisory council within the Hannah Penn K-8 School
- Provide administrative support for school-based health and Connections functions, as needed
- Perform other duties as assigned
KNOWLEDGE/SKILLS/ABILITIES
- High School Diploma or equivalent
- Preferred combination of relevant education, training, and/or experience
- Proficient with the use of Microsoft Office (Word, PowerPoint, Excel, Outlook)
- Understanding of basic community engagement, including ability to exhibit excellent working relations with patients, stakeholders, visitors and staff
- Creative problem-solving skills Self-starter with ability to work independently Good organizational skills to handle multiple priorities in a deadline-driven environment
- Excellent written and verbal communication skills
- Ability to support community outreach
- Ability to be flexible
- Ability to work with many diverse people, including children and teenagers
- Ability to critically think clearly and rationally about what to do or what to believe; understand logical connections between ideas; and identify, construct, and evaluate arguments
