What are the responsibilities and job description for the Client Care Coordinator position at Family Haven?
The client care coordinator plays a vital role in helping clients feel safe and consistently taken care of during their time with Family Haven. They work closely with the clinical team to ensure clients’ needs are met from their first phone call throughout their treatment.
A typical day as Family Haven’s client care coordinator could include many of the following things:
- communicating with prospective clients about their therapy needs and helping them determine if Family Haven has a clinical and financial fit for them,
- working with community partners and office staff to find short or long term assistance for new or existing clients,
- communicating with case workers regarding services for DCFS families,
- setting up new client records in the HRM system,
- working with the clinical director to assign therapist caseloads or
- helping a current client understand their financial responsibility for treatment
The ideal candidate is able to prioritize tasks and keep track of several different clients in different stages of onboarding, has experience working in an office setting and great time-management skills as most of the work is done independently.
RESPONSIBILITIES
- Works with community partners and contracts to get referrals for treatment
- Ensures all clients are eligible for services and have all needed documentation and consent to start therapy
- Screen clients for grant participation and refer clients to the grant application process as needed
- Works with office and clinical staff to ensure continuity of care by adjusting schedules or funding.
- Ensures quality of client records by auditing clinical notes and treatment plans and ensuring records are closed when necessary
- Creates client records, set up billing cases, client profiles, and schedule the first appointment with a new client in HRM.
- Communicates with clients regarding changes to their insurance or other information.
- Coordinate treatment authorizations with DCFS case managers or other community partners
- Coordinates insurance paneling and re-attestation for therapists
- Helps clients become aware of and connected with in-agency resources
- Maintains strict confidentiality with regards to all client and clinical information.
PREFERRED QUALIFICATIONS
- Higher education in social work, psychology or related major OR experience working in case management, client coordination or mental health.
- Proficient with Excel
- Familiarity with HRM systems
- Experience with health insurance billing or coordination of benefits.
- Ability to speak Spanish
- Experience with trauma-informed care
