The physician provides direct preventive and primary care for pediatric, adults and geriatric patients. Family Health Center is a Federally Qualified Healthcare Center (FQHC) that provides comprehensive, integrated primary health care services for all, regardless of one’s background or circumstances. Care provided is related to health maintenance, acute care, and chronic disease management at the health center or in outreach settings.

JOB QUALIFICATIONS

EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Minimum Required: Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.).

Preferred/Optional: None

EXPERIENCE

Minimum Required: Ability to relate to culturally diverse patients and communities.

Preferred/Optional: One to three years' experience in Family Practice. Experience working in a Federally Qualified Health Clinic (FQHC), Community Health Clinic (CHC), or Rural Health Clinic (RHC).  

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.

Minimum Required:

 · Board certified by the American Board of Medical Specialties (ABMS) or the American Osteopathic Board of Medical Specialties (AOBMS)

· Successful completion of an accredited Family Medicine or Internal Medicine Residency and currently Board certified or Board eligible in your specialty.

 · Current and unrestricted applicable Wisconsin MD/DO license
