SUMMARY:

The role of the BH Care Coordinator is to assist both the Integrated BH team and BH Specialty team with support services that ensure the continuity of care for patients.


$22.00 - $25.00 DOE.


Employees can receive up to $520 on the quarterly bonus.


The BH Care Coordinator provides behavioral health case management and service coordination for patients receiving services at our behavioral health clinics. BH Care Coordinator works with medical teams in our rural clinics to identify patients who might benefit from integrated behavioral health appointments via telehealth, help explain telehealth process to the patient and transition care to the Behavioral Health Consultant. The care coordinator role also involves development of patient registries and initiation of on-going contact with identified patients until gains are made on their BH goals.


MINIMUM QUALIFICATIONS:

Bachelor degree in a social science field required. Current State of Idaho Social Worker license in good standing preferred.


KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

· Knowledge and understanding of case management methods, procedures, and standards for children and adults.

· Interviewing skills to complete needs assessments and goal planning.

· Skills in preparing and maintaining patient records.

· Ability to learn and work with electronic health records and Microsoft Office.

· Knowledge of community resources.

· Ability to create, compose, and edit written materials.

· Ability to make procedural decisions and judgments.

· Ability to maintain emotional stability to cope with human suffering, emergencies, and other stresses.

· Strong communication, problem-solving and interpersonal skills.

· Ability to work effectively with a wide range of constituencies in a diverse community.

· Ability to use language lines and interpreters when needed.

· Ability to evaluate the progress of services being provided and communicate concerns or recommendations to supervisor.

· Ability to multitask and provide instruction/guidance to others.

· Knowledge of legal and ethical issues related to patients' rights.


DESCRIPTION OF DUTIES:

1. Collaborates with supervisor and care coordination team in reviewing progress toward program goals, adherence to program service models and work flows.

2. Collaborate with FHS employees to ensure programs are working together, not duplicating services and problem solving any barriers that have been identified.

3. Is available to primary care patients and health professionals on a same-day basis and accessible by phone during working hours.

4. Manages referrals from FHS staff for patients who need additional resources or services.

5. Helps team identify when standard screening and outcome measure instruments are needed, can help administer these if appropriate to scope of the position.

6. Provides support to the PCBH pathway program (e.g., constructing and maintaining registries, preparing evaluation summaries).

7. Provides referrals, patient education and community resource materials to patients when appropriate and within scope of the position.

8. Contact adults and families who have requested BH case management services following the required process for assessing need, service delivery and documentation.

9. Provides service coordination to FHS behavioral health patients who have been referred to additional services, following up to identify if there are barriers to accessing those services and identifying any resources that may help address those barriers.

10. Regular meetings and communication with supervisor to review and consult on patients involved in case management and service coordination.

11. Performs miscellaneous job-related duties as assigned.

FHS employees will do their part to ensure accurate documenting and billing practices, participate in compliance trainings and will identify and report any concerns or activities that may violate these standards.

Each employee will be trained on the FHS Medicare Compliance Plan and the Standards of Conduct and asked to sign a Conflict of Interest Statement at hire and annually thereafter. PATIENT CENTERED MEDICAL HOME (PCMH):

Family Health Services is committed to providing our patients with the highest standards of care by becoming recognized as patient centered medical home. FHS staff is expected to participate in this process by being an active and willing PCMH team member. Specific duties and expectations may vary and will be identified by position and site.


PROCEDURE COMPLIANCE:

Employee must read and understand the general and specific operational, safety, and environmental requirements of all plans, procedures, and policies pertaining to this job.


WORKING CONDITIONS:

Work is normally performed in a typical interior/office work environment. No or very limited physical effort required. No or very limited exposure to physical risk.
