JOB OVERVIEW:

The Licensed Clinical Social Worker (LCSW) develops treatment plans and implements preventive strategies for Northeast Florida Health Services (NEFHS) patients experiencing behavioral or mental health conditions.


ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

  • Conduct individual therapy sessions using evidence-based therapeutic approaches such as cognitive-behavioral therapy (CBT), motivational interviewing, and psychopharmacology tailored to the needs of patients within a Federally Qualified Health Center (FQHC) setting.
  • Perform mental health assessments and diagnosis, integrating findings into comprehensive treatment plans as necessary.
  • Administer psychosocial assessments upon patient admission, annually, and as clinically indicated to evaluate social, emotional, and physical needs, identifying and addressing barriers to care within the FQHC model.
  • Independently manage a caseload of patients with varying clinical acuity, ensuring timely and effective interventions and follow-ups.
  • Provide comprehensive assessments and diagnoses for patients across the lifespan, considering the unique challenges and circumstances faced by individuals in underserved communities.
  • Collaborate closely with interdisciplinary teams within the FQHC to coordinate care and ensure holistic treatment approaches for patients.
  • Maintain accurate and up-to-date medical records in compliance with FQHC standards and regulations.
  • Advocate for patients' rights and access to resources within the community, including facilitating referrals to external agencies as needed.
  • Offer crisis intervention services as required, demonstrating responsiveness and adaptability to urgent patient needs.
  • Leverage experience working specifically within the FQHC model to address the healthcare needs of diverse patient populations and underserved communities.
  • Prepare detailed social case histories emphasizing psycho-social factors affecting patients, collaborating with staff and community agencies to optimize treatment outcomes.
  • Stay abreast of new techniques and best practices in addressing emotional and environmental challenges within the context of FQHC care delivery.
  • Perform additional related duties as required to support the ongoing needs of NEFHS.


EDUCATION/EXPERIENCE:

  • Master’s degree in Social Work (MSW) from an accredited program.
  • Proven experience working in a clinical setting, with a strong preference for candidates who have worked within Federally Qualified Health Centers (FQHCs) or similar primary care outpatient clinics.


QUALIFICATIONS/SKILLS:

  • Demonstrated ability to establish rapport and build therapeutic alliances with patients from diverse backgrounds, exhibiting excellent interpersonal skills.
  • Strong assessment and diagnostic skills.
  • Knowledge of behavioral health and the skills to assess mental health functioning, high-risk behaviors, depression, anxiety, or other psychiatric conditions impacting hospitalization or transitions of care.
  • Proficiency in conducting comprehensive assessments and diagnoses, with a focus on addressing mental health functioning, high-risk behaviors, substance use disorders, and other psychiatric conditions commonly encountered in FQHC settings.
  • Sound knowledge of behavioral health principles and evidence-based interventions, coupled with the ability to develop and implement effective treatment plans tailored to individual patient needs.
  • Knowledge and understanding of the influence of cultural and spiritual values in social work practice.
  • Strong ability to provide psychosocial care appropriate to the age-specific needs.
  • Proficient in using a computer to accurately enter and extract data, send and receive email, calendar appointments, and use task lists as will be using a variety of computer software programs.
