SUMMARY
Family Service League is seeking a part-time Nutritionist/Dietitian for the Long Island Sound Senior Connections (LISSC) Program. The Nutritionist will provide nutritional counseling and education for clients over 60 years of age. The Nutritionist will work with clients in developing a custom nutritional meal plan tailored to each client's dietary restriction and needs.
RESPONSIBILITIES
- The Nutritionist will conduct in-home nutritional assessments for each client.
- Counsels clients on various dietary concerns while promoting healthy, nutritional eating habits.
- Works closely with patients in developing a sustainable and customized nutrition plan that aligns with their health goals.
- The Nutritionist will follow up with patients to evaluate progress and efficacy of designed nutritional plans, making adjustments as necessary.
- Creates and delivers educational materials and resources regarding nutrition and diet.
- The Nutritionist will ensure compliance with necessary documentation and obtain proper consent forms in order to coordinate care.
- Maintains detailed records that are up to date and entered in the Electronic Health Record, timely.
- All other duties as assigned.
QUALIFICATIONS
Bachelor's degree in Nutrition or Dietetics required.
Licensed by New York State as a Nutritionist or Dietitian required.
Must possess a minimum of 3 years of experience working with the senior/geriatric population.
Excellent interpersonal and verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required.
Experience working in an electronic health records system preferred.
Valid and clean NYS Driver’s License required.
PHYSICAL REQUIREMENTS
Sitting for long periods of time and traveling to conduct home visits.
