Job Description
FamilyTIES of SC, LLC is a Case Management provider for CLTC (Community Long Term Care). As case managers, we are focus on helping our Participants remain at home, in the community with their family and friends. Our goal is ensuring our Participants receive appropriate individualized services, provide service counseling, and support!
We are looking for case managers to serve the Florence, Horry and Richland area. Primary focus is to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate(s) will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client's health and human services needed. A certified case manager is a plus. MUST HAVE AT LEAST 1-2 years of case manager experience and a bachelor degree from an accredited college or university in a health or human services field that promotes the physical, psychosocial, and/or vocational well-being of the individual being served.
