FAYCO Enterprises, Inc., a CARF accredited not-for-profit, is looking to fill the position of a Rehabilitation Services Coordinator. This is a full-time position working Monday thru Friday in Vandalia, IL.
The successful applicant will ensure that case coordination, intake and evaluation services are provided as required by funding source, administer assessments (SIBS, ICAP, McCarron Dial) as required, develop written evaluation plan & evaluation reports with recommendations, procedures and materials for individuals served. Responsible for supervising Work Partners and Future Bound program and staff.

REQUIREMENTS
*Bachelor degree in related field
*Must meet federal definition of QIDP (Qualified Intellectual Disabilities Professional)
*One year experience with individuals with intellectual/developmental disabilities
*Be at least 21 years old
*Must have a valid driver’s license & reliable transportation
*Successfully complete assessment training within 120 days of hire
*Previous supervisory and computer experience a plus
Starting at $22.50 per hour
Generous sign-on and referral bonus program of your choice - $500 for 6 months OR $1000 for 1 year
