What are the responsibilities and job description for the Treatment Coordinator position at FCC Behavioral Health?

Job Purpose

The services provided by a Treatment Coordinator in the Healing Council: Recovery Support Services for Adults in Treatment Court focus upon stabilizing clients already being served by drug court treatment teams who are identified as being at-risk, as well as ensuring that these referred clients have safe housing, adequate insurance, and gainful employment. During these stabilization interventions, Treatment Coordinators will teach, model and practice skills with person(s)-served in order to promote recovery and wellness.

Qualifications

  • A mental health professional, OR
  • Bachelor’s degree in social work, psychology, nursing, or human services field, OR
  • At least two years of higher education with two years of experience in psychiatric, substance use treatment or developmental disabilities, OR
  • Four years of equivalent experience with consumers and their families receiving psychiatric, substance use treatment or developmental disabilities services to move towards their personal, social, and vocational competency in order to live successfully in the community, OR
  • At least a high school diploma or equivalent and applicable training and testing as required by the Department of Mental Health and be supervised by a qualified mental health professional as defined in the Code of State Regulations. Lived experience with behavioral health and /or substance use and/or co-occurring treatment and justice involvement.

Core Job Duties

  • Identify strengths and goals of person(s) served.
  • Assist person(s) served in making independent choices and being proactive in treatment.
  • Assist person(s) served in overcoming barriers to successful completion of Treatment Court and transition back into community living.
  • Ensure person(s) served has access to all available behavioral health services, including community support, peer support, individual, family, and/or group therapy, medication management, metabolic syndrome screening, and infectious disease testing with referral to providers as needed.
  • Link person(s) served with community resources to reduce recidivism, obtain safe and supportive housing, obtain health insurance, and engage in gainful employment.
  • Provide education to person(s) served about managing symptoms of substance use disorder and/or mental illness.
  • Help person(s) served develop or improve interpersonal, social, family, community, and independent living skills.
  • Provide transportation for the person(s) served when necessary for critical treatment interventions.
  • Actively attend and participate in treatment team meetings.
  • Work with project manager and other project staff to develop and implement outreach strategies and referral pathways.
  • Work with project manager and other project staff to gather relevant data and generate reports as needed.
  • Communicate information effectively and accurately both orally and in written form.
  • Complete required documentation in a timely manner.
  • Adherence to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.
