Summary:
Working as part of a team, the Coordinated Access Navigator will conduct short term case management for clients experiencing, or at risk of entering, homelessness throughout Sacramento County. Navigators report directly to the Coordinated Access Navigator (CAN) Program Manager. Navigators will work to build relationships with persons experiencing homelessness and assist with connection to long-term case management, housing, and shelter services. Navigators will respond to referrals from Sacramentos 2-1-1 and provide persons experiencing homelessness with short-term case management. The Navigators will also connect with clients through street outreach and engagement with individuals in and around homeless encampments. Navigators primary focus will be building rapport with persons experiencing homelessness and guiding them to positive outcomes through service
and shelter connection. Navigators will enter client and service provision information and data into the Homeless Management Information System (HMIS). Essential Duties and Responsibilities: Include the following, specific duties may vary. Other duties as

Duties:
The Coordinated Access Navigator is responsible for managing their caseload and connecting persons experiencing homelessness to long-term services.
1. Actively manage and coordinate service provision for all clients assigned to caseload.
2. Build rapport, provide support, and empower clients to be part of their own success.
3. Work collaboratively with partner agencies to conduct outreach and client connection in
encampments and other community based settings.
4. Ensures all clients receive prompt and appropriate case management services.
5. Work in various environments, including but not limited to, partner agencies, drop-in centers,
homeless shelters, and homeless encampments.
6. Complete periodic chart audits monthly or as specified, returning the audit checklist to your director.
7. Conduct needs assessments for clients.
8. Appropriately assess client needs and provide relevant service referrals and connections.
9. Accurately input all services provided to and contacts with clients into HMIS.
10. Manage the referrals to service and progress of multiple clients simultaneously.
11. Works collaboratively and as part of a team.
Qualifications
- Cultural awareness and competency skills.
- Comfortable working with diverse populations including LGBT, individuals experiencing homelessness, and individuals struggling with mental illness and or addiction.
- Ability to connect and engage with people
- Excellent verbal and written communication skills with ability to read, write, speak and understand English clearly.
- Detail oriented, organized and possess time management skills
- Strong interpersonal skills with a demonstrated ability to work independently on individual and assigned tasks while exhibiting good judgment.
- Provide information in a manner that is culturally and linguistically appropriate to the needs of the client.
- Ability to work as a team member including in co-location settings such as clinics with clinicians and professionals as well as other community organizations.
- Serve as a resource for clients, other navigators and clinical staff.
- Working knowledge of Microsoft Office applications preferred.
- Problem solving, decision making and critical thinking skills required.
- Must have reliable transportation and be able to travel within the County.
- Must have a valid drivers license and pass a background/fingerprint check.
Requirements:
High School diploma or GED required. Bachelors degree in social work or a related field
preferred.
Experience working with unsheltered individuals experiencing complex issues is desirable.
Experience in outreach, case management, and/or familiarity with homeless service provision in
the Sacramento Continuum of Care is preferred.
Must have valid drivers license and good driving record.
All offered employees must submit to a background check.
Client Case Load:
The Coordinated Access Navigators will maintain a case load of approximately 25 clients at any given time, as staffing permits.
Salary : $24 - $27
