Job Summary
Community Social Worker
Position Purposes: The Community Social Worker provides affirming, respectful, and supportive care to individuals from diverse backgrounds and lived experiences. The Community Social Worker is responsible for conducting biopsychosocial assessments and case plans, providing case management services, and working as part of the interdisciplinary health team for clients receiving care in rural clinic locations at Five Horizons Health Services (FHHS). The Community Social Worker will also conduct outreach within FHHS’s coverage area by providing linkage to services for off-site clients and developing relationships with community partners.
Essential Functions:
·Provide direct social work services to a caseload of clients at different levels of care as dictated by their care plans to facilitate client retention in medical care
· Represent Five Horizons Health Services in the community and develop strong connections with local organizations and resources throughout the rural coverage area
·Serve as the primary contact for clients receiving care in rural clinic locations
·Travel between multiple rural clinic locations
·Maintain a strong knowledge of available services to include Ryan White HIV / AIDS Program (RWHAP) Part B, AIDS Drug Assistance Program (ADAP), and Alabama Insurance Assistance Program (AIAP)
·Demonstrate a skilled knowledge of engagement, assessment, case planning, and monitoring techniques to maintain the helping relationship between the client and FHHS
·Promote client participation in the healthcare continuum through face-to-face encounters, telephone interactions, text messaging, and any other engagement techniques as needed
·Maintain required client records, document services provided, scan client documents into EHR, and complete other required paperwork in a timely manner
·Maintain compliance with HIPAA and state guidelines as well as FHHS policy and procedures
·Perform any other department / agency-related duties or special projects as directed by supervisor
·As with all other positions, during emergencies, the Community Social Worker is expected to fill in at any position within the agency for which they are capable of performing
Education/Professional:
· Bachelor’s or master’s degree in social work
· Licensure or license-eligible at the appropriate level through the Alabama State Board of Social Work Examiners
· Must possess a strong ability to interact with diverse groups and cultures
·Strong organizational, time management, verbal communication, and written communication skills
· Must possess a valid driver license and confirm effective automobile insurance coverage
Physical Requirements: This position requires an individual who is self-motivated and able to independently complete assigned responsibilities. Moreover, this individual must also be able to accept proper supervision and work cooperatively with clients, peers, and other professionals.
Frequent sitting, some standing / walking, and occasionally lifting supplies / equipment up to 50 lbs. Occasional reaching, bending, stooping, kneeling and crouching. This position requires an individual who is mentally alert and able to effectively communicate verbally and in writing. Individual must be able to effectively utilize
Microsoft Office programming and, after necessary training, electronic health records system and Telemedicine equipment.

Pay: From $47,000.00 per year
