Position Overview
The Population Health Manager will provide on-site supervision and monitoring of the Care Management Program. The Manager monitors staff assignments including assigning caseloads, initiation of care plans, and monitoring health outcomes. This position will work closely with the QI and Transformative Health Manager and the Center Managers.
General Responsibilities
- Will carry own caseload to assess patient's clinical needs.
- Develop and implement nursing care plans.
- Utilize electronic medical records and database management systems to identify high -risk, high-volume patient populations.
- Advises unlicensed staff on how to direct patients on health maintenance and disease prevention or provide care management.
- Assist in the management of high-risk, complex patients.
- Identify the psychological, behavioral, emotional and social factors important to the prevention, treatment or management of physical health problems.
- Develop referral protocols and agreements with community agencies.
- Discuss self-management goals with the patient and/or family and assist them in selecting appropriate goals.
- Ability to work on-site at multiple locations; local travel required.
Qualifications
- Graduation from an accredited school of nursing required
- Registered Nurse License in the state of Ohio required
- 5 years experience in an office setting preferred
- Windows-based computer experience required
- EPIC experience required
