Director of Patient Care Services
POSITION SUMMARY:
Responsible for the overall care delivery, patient experience and supervision of clinical staff in the agency
RESPONSIBILITIES:
- Coordinate and oversee all direct and indirect patient services
- Supervise clinical staff effectively and complete performance actions
- Identify and report any unusual occurrence, accidents or adverse effects of medical devices
- Assist with follow up review and documentation
- Provide leadership in the assessment of patients and their families
- Take referrals and ensures that assessment visits are made
- Responsible for the efficient assignment, scheduling and supervision of clinical staff
- Review medical records routinely and conduct clinical record reviews to supervise the quality and timeliness of care and documentation.
- Investigate adverse events to identify opportunities to improve care
QUALIFICATIONS:
- A registered nurse with three years of experience in a home health agency with experience in a supervisory or administrative capacity
- Excellent interpersonal, verbal and written communication skills and ability to recognize the needs and concerns of people and maintain constructive working relationships with them
- Must have a vehicle, valid driver’s license and automobile insurance
- Ability to make home visits and provide skilled nursing assessments and care in the home environment

Salary : $130,000 - $140,000
