**Job Summary:**
FTPO is seeking a compassionate and dedicated PRP (Psychiatric Rehabilitation Program) Counselor to join our team. The PRP Counselor will be responsible for providing individualized support and rehabilitation services to clients with mental health challenges, helping them achieve their personal goals and improve their quality of life.
**Key Responsibilities:**
- Conduct comprehensive assessments to identify clients’ strengths, needs, abilities, and preferences.
- Develop and implement Individualized Rehabilitation Plans (IRPs) in collaboration with clients, setting achievable short-term and long-term goals.
- Provide one-on-one counseling and support to clients, focusing on areas such as medication management, wellness, home maintenance, financial stability, mood regulation, and employment.
- Facilitate group therapy sessions and wellness activities.
- Monitor and document clients' progress, adjusting plans and interventions as needed.
- Advocate for clients' needs and coordinate with other service providers and community resources.
- Maintain accurate and confidential client records in compliance with CARF standards and organizational policies.
- Participate in ongoing training and professional development opportunities.
**Qualifications:**
- Minimum of 2 years of experience in mental health counseling or psychiatric rehabilitation.
- Knowledge of CARF standards and experience in developing IRPs.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient, with a genuine desire to help others.
- Proficient in Microsoft Office and electronic health records (EHR) systems.
- Valid driver’s license and reliable transportation.
