What are the responsibilities and job description for the Psych-Social Assistant position at Forest City Rehab and Nursing Center?
JOB SUMMARY: The purpose of this position is to conduct and document group and one-to-one programs, meeting the psycho-social needs of the residents and demonstrating progress toward their goals.
QUALIFICATIONS:
- Must have a high school diploma.
- Demonstrate adequate English oral and writing skills.
- Must have a high level of organizational skills, initiative, creativity, and communicate well with others.
JOB REQUIREMENTS:
- Must be physically and mentally capable of performing routine, repetitive job duties.
- Uses skill and understanding in handling residents.
- Be considerate, pleasant, and professional in the performance of his/her duties.
- Must have compassion, tolerance, and understanding for the elderly.
- Knowledge of JCAHO, OBRA, IDPH, and HFS regulations.
MAIN DUTIES:
- Support the facility's philosophy of care and strive to achieve its goals and objectives.
- Meet with groups three (3) times a week according to schedule.
- Carry out one-on-one intervention three (3) times a week according to schedule.
- Follow schedule for group and one-to-one programs.
- Write monthly progress notes in a timely fashion.
- Chart daily attendance in group activities.
- Prepare and complete monthly lesson plans.
- Follow through with approach and goals developed.
- Identify and communicate with Psycho-Social Director regarding any needed change in goals, programs, schedule, etc.
- Complete care plan as needed.
- Organize paperwork including:
- a) schedule of program meeting times and places
- b) list of one-on-ones and group members
- c) group protocols
- d) attendance
- e) current goals or copies of evaluations and reassessments as needed
- f) lesson plan
- Assure that work schedules are followed.
- Maintain the confidentiality of resident information and honor his/her personal and property rights.
- Attend Psycho-Social department meetings and others as directed.
- Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed.
- Turn in Day Off Request forms, supply sheets, and annual medical examinations in a timely fashion.
- Follow established fire, disaster, safety, infection control, and evacuation policies and procedures.
- Perform other duties as assigned by the Director of Psycho-Social Services and Administrator.
