We are seeking a qualified Case Worker (Bilingual) with a strong commitment to social services and older adults to work at 4 Center Based Sites (Forest Hills, Jamaica, Kew Gardens, and Pomonok Center). Services include case assistance, benefits counseling, completion of benefit applications and recertifications, providing information and referrals, registering new clients, and making telephone reassurance calls. The Caseworker will assist the center staff in building a supportive community for older adults through needs and eligibility assessments, participant advocacy, and partnership building.

Primary Responsibilities:

  • Interview clients to determine problems they encounter, including personal/family adjustments, finances, and physical/mental impairments, to determine the nature and degree of the problem.
  • Provide accurate and appropriate information on services, benefits, and entitlements to clients.
  • Conduct on-site visits with new program participants to assist with engagement with other program members.
  • Help clients complete application and recertification processes.
  • Develop case notes, create action items for care plans to reflect client assessments, and follow-up to determine action outcomes.
  • Perform new client registrations.
  • Make telephone reassurance calls.
  • Consult with program directors to ensure that client needs are met reflecting culturally sensitive options that make efficient use of community resources.
  • Develop and maintain partnerships with other agencies to expand resources; advocate and intervene with other agencies on behalf of participants.
  • Participate in meetings, training, and staff development activities.
  • Attend conferences/ trainings in-house and at other agencies.
  • All other duties as assigned.

Qualifications:

  • Bachelor’s degree in Social Work, Sociology, Human Services, or related field preferred.
  • Minimum of two (2) years of experience working with older adults or case management senior services sector.
  • Knowledge of benefits/entitlements experience is a plus.
  • Superior written and verbal communication skills.
  • Excellent interpersonal skills with a demonstrated ability to build and sustain relationships.
  • Self-motivated and resourceful, able to work independently as well as collaboratively with staff.
  • Ability to maintain confidentiality and use appropriate discretion.
  • Proficient in Microsoft Office applications including Word, Excel, and Outlook and database management/search tools.
  • Bilingual-Spanish verbal and written competencies.

Status: Full-Time, Non-Exempt

Salary: $24.73

Schedule: 35 hours per week, Monday – Friday, 8:30 am – 4:30 pm
