Primary Duties and Functions:
The licensed practical nurse (LPN) Care Manager (CM) will coordinate with the care team to oversee a plan of care for patients living at home. The LPN CM will evaluate the environment, functional and psychosocial status, and financial situation of a patient, consult their physician, and assist in the establishment of an action plan that empowers the patient to remain as independent as possible in the community. The LPN CM clarifies treatment plans, monitors medication regimens, and addresses other urgent issues. The LPN CM will also document care and improvement and provide support to family members. This individual will be an integral part of the care team, supporting the RN care manager in meeting the needs of the patient. The LPN CM will collaborate with other professionals and do whatever is needed to coordinate the care of the patient.
