Job Description
Fort Smith Regional Dialysis Center is seeking a Full Time Social Worker. The Center is located in Fort Smith, Arkansas and is privately owned.
The Social Worker Master's Level is responsible for participating in the evaluation and assessment of Hemodialysis and Peritoneal Dialysis patient needs. Assists and educates patients and their families in the availability of and application for various resources, including basic social and physical needs. Works with patients and their families, staff, physicians, and other staff to locate resources to remove barriers to patient treatment. Monitors patients, assisting with any problems that may arise, making referrals as necessary. Initiates and participates in patient and family education. Assists patients with emotional crises, grief, and end-of-life issues as required.
Monday – Friday
8am – 5pm
No weekends, No call
