What are the responsibilities and job description for the Program Coordinator - Outpatient Services - Full Time position at Fremont Hospital?
The Program Coordinator plays a critical role in supporting clinical operations—particularly within acute services—while coordinating care and advocating for patients and their families as they navigate the complexities of the hospital and broader healthcare system. This position requires a strong understanding of acute services, including partial hospitalization programs (PHP), intensive outpatient programs (IOP), chemical dependency services, and inpatient levels of care. The Program Coordinator’s primary goals are to increase patient satisfaction and optimize care delivery and outcomes across Fremont Hospital’s outpatient services. Serving as a communication liaison, the Program Coordinator helps identify each patient’s clinical needs, preferences, and concerns. They connect patients and families with appropriate services, resources, and information to support healing and enhance the overall care experience. The role involves regular interaction—by phone and in person—with a diverse range of clinical populations and includes coordinating care across multiple service lines within the hospital.
In addition to clinical coordination, the Program Coordinator also supports the day-to-day operations of the outpatient office, functioning in an office manager capacity. This includes covering the front desk, welcoming and checking in patients, managing incoming calls, ensuring smooth office workflow, maintaining supplies, supporting scheduling, and overseeing general administrative needs. This role serves as the first point of contact for many patients and families and is essential to maintaining a professional, organized, and responsive outpatient environment. Working under the guidance of the Executive Director of Business Development, the Program Coordinator receives direction and support to ensure programmatic goals align with broader strategic initiatives.
