Purpose and Scope: As a registered professional nurse in our Home Therapies department, you will provide care to patients undergoing dialysis treatment. This includes assessing patient needs, developing individualized care plans, and collaborating with the healthcare team to ensure optimal patient outcomes.
Key Responsibilities:
- Assess and manage patient response to home dialysis training and treatment therapy
- Communicate patient-related issues to physicians as needed
- Participate in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations
- Perform ongoing, systematic collection and analysis of dialysis data for assigned patients
- Direct and provide all aspects of the provision of safe and effective delivery of dialysis therapy to assigned patients
Requirements:
- Graduate of an accredited School of Nursing
- Minimum 1 year experience as a Registered Nurse (preferred)
- Home dialysis therapy experience (preferred)
