Position Title: Case Manager

Reports to: Division Director-La Casa

Location: 1444 Irving Street NW, Washington, DC

Hours: Flexible hours between 8:00 AM – 6:00 PM with occasional evening work. Participates in an on-call rotation.

FSLA Status: Exempt, 1.0 FTE

Classification: Safety Sensitive

Summary:

The La Casa PSH Program is a Permanent Supportive Housing program in the District, which contains 40 studio-sized apartments in a state-of-the-art building.  The program provides comprehensive case management services.  The Permanent Supportive Housing program follows a Housing First model philosophy and utilizes best practices to ensure participant success. The Case Manager provides direct case management services to previously chronically homeless single adult males living in single-site Permanent Supportive Housing. The Case Manager will assist participants in identifying and achieving immediate and long-term goals that will allow them to stabilize and remain in permanent housing.  Case managers will work to empower participants of the housing program while promoting continued growth. The Case Manager will deliver participant-centered services to a caseload of at least 19 men who live on-site.  


Essential Duties and Responsibilities:

  • Provide supportive, participant-centered, trauma-informed case management services to recently homeless adults with serious mental illnesses, addictions, medical needs, etc. 
  • Complete intake assessments and the DHS application with new participants and provide an orientation to the building and program. 
  • Assist participants in developing community-based support networks. 
  • Deliver participant-centered face-to-face contact to residents in both individual and group settings. 
  • Foster wellness and independence and demonstrate empathy and flexibility with participants. 
  • Engage participants and assess individual needs, preferences, and strengths. 
  • Respond to immediate participant needs—assisting in crisis situations, resolving conflicts, and meeting basic needs. Report incidents in accordance with agency policy. 
  • Develop, implement, and revise individual service plans with each resident on caseload for the “Housing the Homeless database system” (OCTO). Document all contacts made with caseload into the OCTO database system at least twice monthly and input notes in Credible.
  • Provide comprehensive case notes that reflect case plan goal-oriented updates and status into a database system. 
  • Maintain consumer electronic records by collecting data and completing clinically appropriate progress notes. 
  • Monitor medical and psychiatric needs and assist caseload with medication management. 
  • Prepare bio-psychosocial assessment for clinician review. 
  • Collaborate with external agencies providing services to participants within the restraints of HIPAA. 
  • Collaborate through treatment team meetings, weekly calls, and regular follow-ups with Community Agencies. 
  • Form and maintain positive relationships with the community and stakeholders. 
  • Participate in multidisciplinary meetings and regular clinical supervision. 
  • Attend agency training, TCP, DHS training, and conferences as needed or required. 
  • Perform all other duties as assigned.

Requirements

Education/Experience:

Bachelor’s degree in human services or related field with 2 years’ experience in a similar setting.  

Experience with addictions and mental health is preferred. 


Qualifications:

Strong written and verbal skills 

Ability and willingness to work in a diverse and challenging environment.  

Ability to work as a team player and maintain flexibility. 

Passionate about providing a broad spectrum of services to vulnerable populations. 

Familiarity with the Housing First Model and Motivational Interviewing. 

Proficient in Microsoft Word, Excel, the Internet, etc. “Housing the Homeless Database (OCTO) and Homeless Management Information System (HMIS) experience a plus. 


Working conditions:

Employee will work during normal work hours with occasional evening and weekend work.  Employee will participate in on-call rotation, Work in residents’ homes, and escort residents to appointments as needed by company vehicle.  


Physical and Environmental Requirements:

Occasional driving and community work.  Ability to lift at least 20 pounds. 


Background checks: 

The selected candidate will be required to complete and pass background checks specific to contract requirements that may include criminal history background, FBI Background, DC Metropolitan Police Background Check, State-of-Residence Police Background Check, DC Child Abuse Registry (CFSA), National Sex Offender Registry Check, Drug Panel and Alcohol Screening, Tuberculosis Test, and Driving Record check (for positions transporting participants).

The schedule is to be determined and is subject to change. Some evenings, weekends, and holidays may be required. Participation in an emergency on-call rotation may be required.

Compensation: The hiring pay range is between$58,117.00 - $63,506.00 ­­­­­­­­­­­­­. The starting annual salary is based on a step system. A candidate’s starting pay determination is based on meeting the basic position requirements and completing education, credentials, and work experience. Please Note: The step system is set and not open to pay negotiation.
