Job Summary
Gadsden Health and Rehab is seeking a dynamic and compassionate Admission Nurse to join our healthcare team, dedicated to providing exceptional patient care from the moment of admission. This job is Monday through Friday from 930AM to 6PM. In this vital role, you will serve as one of the first points of contact for patients entering our facility, ensuring a smooth, efficient, and welcoming intake process. Your expertise will help facilitate accurate assessments, comprehensive documentation, and seamless coordination with medical teams to support optimal patient outcomes. Responsibilities
- Conduct thorough patient assessments upon admission, including vital signs, medical history review, and initial physical examinations.
- Collect and document accurate patient information using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Coordinate with multidisciplinary teams to facilitate appropriate admissions for various specialties including critical care, emergency medicine, geriatrics, and outpatient services.
- Prepare and verify necessary documentation for insurance processing, including ICD-10 coding and CPT coding when applicable.
- Assist with specimen collection and processing, phlebotomy, and administering injections as needed for initial evaluations.
- Educate patients and families about nursing home procedures, therapy, discharge planning, and post-admission care instructions.
- Maintain sterile techniques during procedures such as suturing or catheter care while adhering to infection control protocols.
Skills
- Proven experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks.
- Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods.
- Skilled in patient assessment techniques including vital signs measurement and physical examinations across diverse populations including pediatrics and geriatrics.
- Proficiency in IV infusion, venipuncture (venipuncture), catheterization (including urinary catheters), and aseptic technique.
- Experience working with complex patient populations such as those requiring rehab, long-term care, hospice & palliative medicine.
- Ability to handle emergency situations with calm professionalism; experience in triage or urgent care settings is highly desirable.
- Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for billing purposes, and medical documentation standards aligned with NCQA standards.
- Excellent communication skills to collaborate effectively with healthcare teams and educate patients about their care plans.
