What are the responsibilities and job description for the On-Call RN Clinical Educator - Home Infusion Pharmacy position at Gaspy Infusion Services, Inc.?
Gaspy Infusion Services is seeking a PRN RN to support our Houston, TX location. Clinical Educator will be responsible for the initial assessment and education of patients and caregivers on the initiation of their home infusion therapies.
This role involves visiting patients at hospital bedside and occasionally at home around the Houston area and provide initial education and initial hook ups
Monday-Friday
- Provide direct care for all specifically assigned patients for initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members.
- Evaluate and educate patient/caregiver to safely administer the home infusion therapy and provide additional instruction as needed.
- Coordinate care via live telephone conversations and/or voicemail with the pharmacy team as determined by the urgency of information to be relayed.
- Document all communications at the time of service.
QUALIFICATION REQUIREMENTS
- RN Degree, BSN required
- Minimum 2 years nursing experience with background in home infusion therapy
- Curlin pump experience required
- Current nursing license for Texas
- Excellent customer service skills
- must have reliable self transportation and availability to commute around Houston hospital areas.
