Gateways Hospital and Mental Health Center, located in Los Angeles, is a non-profit organization that provides an array of comprehensive mental health services for Children, Adolescents, Transitional Aged Youth, and Adults suffering from mental illness in the Greater Los Angeles area. The organization consists of a standalone fifty-five (55) -bed hospital and six outlying outpatient facilities that have anywhere from fourteen (14) beds to one hundred and thirty-four (134) beds. The organization has approximately four hundred (400) employees.
SUMMARY OF POSITION:
As part of the interdisciplinary Los Angeles County CONREP treatment team, the Case Manager is responsible for guiding clients in the re-integration into the community by coordinating their medical care and supporting their access to treatment. As part of this role, the Case Manager will work closely with the Family Nurse Practitioner to identify and prioritize needed outside medical care for clients. Additionally, the Case Manager will arrange logistical aspects of medical, dental and vision care in the community, such as scheduling appointments, following up with community treatment providers on referrals for specialized services, providing transportation for mobility-impaired clients, serving as an advocate during patient appointments, and coordinating with hospital and urgent care teams.
ESSENTIAL DUTIES:
- Collaborates with the Family Nurse Practitioner to link clients with needed medical, dental, and vision care in the community by scheduling appointments, arranging transportation, transporting clients at times, and advocating for clients during medical appointments.
- Maintains an accurate and comprehensive calendar of appointments with all pertinent information.
- Provides all relevant information to the outside treatment providers to ensure they have information needed to provide informed care.
- Provides clear, timely and comprehensive written and verbal renderings to the treatment team regarding the outcome of appointments and the client’s presentation throughout the appointment, ensuring any change in client’s functioning is reported to all relevant parties.
- Documents services rendered in the EHR in accordance with contractual standards.
- Follows up with outside providers on the status of referrals, prescriptions, and any other treatment related matters.
- Follows up on all hospitalizations, ensuring the hospital treatment team is informed about the clients’ psychiatric condition and medications, as well as his/her legal circumstances. Keeps the treatment team informed regarding care client is receiving and visits the client at the hospital to advocate for the client and provide support.
- Keeps family members informed regarding ongoing medical treatments as indicated.
- Maintains professional and collaborative relationships with outside treatment providers.
- Prioritizes client needs and follows through on tasks based upon these priorities.
- Ensures the company vehicle is properly maintained and completes daily inspection sheets and sends them to appropriate staff.
- Reports safety concerns that arise during medical appointments to the treatment team.
- Participates in annual staffing, routine staff meetings, and emergent meetings.
- Assists in situational crisis intervention as needed.
- Maintains confidentiality of information.
- Performs other additional tasks as directed.
Qualifications
EDUCATION & CERTIFICATES:
Minimum Education:
Associate's Degree in Behavioral Sciences or related area.
Desired Education:
Bachelor’s Degree in Behavioral Sciences or related area.
EXPERIENCE/QUALIFICATIONS:
- Prior case management or medical office experience.
- Academic training in Behavioral Sciences.
- Experience with the mentally ill and/or forensic populations.
- Proficiency in MS Office Suite – Excel, Word, Outlook, PowerPoint, Internet.
- Analytical and problem-solving skills with attention to details.
- Outstanding multi-tasking skills and proven ability to manage multiple projects and work well under time/other constraints.
- Strong communication and presentation skills (both oral and written)
REQUIREMENTS:
- Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
- Valid California Driver’s license.
- TB clearance.
- Driving record acceptable for coverage by Gateways insurance carrier.
- Fire and Safety Training*.
- First Aid Training Certification*.
- CPR Certification*.
- Crisis Prevention Institute Training (CPI)**
* Note: If indicated certifications/licenses are lacking, they must be obtained within 180 days of hire.
**Training needs to be completed within 30 days from the date of hire on a designated date provided by on-boarding. Staff need to be certified at the commensurate level for their role to perform their job duties (staff will be the taken off the schedule if certification is not achieved).
