Job Description
We are seeking a skilled Registered Nurse (RN) to join our hospice team as a Case Manager. As an RN in this role, you will be responsible for delivering high-quality patient care and developing trusting relationships with patients and their families.
The ideal candidate will have a strong background in end-of-life care, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Conduct initial nursing evaluations to determine patient eligibility for hospice services
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Document patient progress and communicate with caregivers and family members
Requirements:
- Licensed registered nurse in good standing
- At least 2 years of experience in hospice or home health care
- Current CPR certification
- Valid driver's license and access to a reliable vehicle
