Starting Salary with benefits: $50,000, based on credentials and experience in skilled nursing healthcare.

Responsibilities of the Social Services Director:
- Greet and orient new residents
- Provide information regarding community resources to residents' families
- Direct hospice and palliative care
- Coordinate discharge planning
- Facilitate regular care plan meetings with residents and their families
- Complete the MDS (Minimum Data Set) within the Assessment Reference date
- Address residents' grievances and assist nursing or other pertinent departments in resolving issues

Requirements:- BA/BS or Master’s Degree in Social Work (MSW) or Licensed Clinical Social Worker
- A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW)
- Minimum two years' experience as a social worker in a skilled nursing or related setting
- A minimum of three (3) years of experience in a supervisory capacity in a hospital, nursing or long-term care facility
- Demonstrated knowledge of the social dynamic of the geriatric population
- Knowledge of community resources
- Strong leadership and relationship building skills
- Working knowledge of government regulations as relevant to social services
