Job Overview
The Triage Nurse delivers general ambulatory nursing care based on health center policies and procedures, working collaboratively with Providers and multidisciplinary team members to assure patient access. This position acts as the point of contact for patients and family to address point in time needs of the patient such as medication refill requests, nurse visits and general care instruction and education. The licensed nurse will promote and restore patient's health as they, provide, delegate, evaluate and coordinate comprehensive professional nursing care through the use of the nursing process for patients of all ages, gender, cultures, and background. The Triage nurse will perform independently to meet patient needs while maintaining a team focused approach to the overall clinic flow.
ESSENTIAL JOB FUNCTIONS:
1. Conducts patient telephone and walk in triage through the use of nursing assessment skills, clinical knowledge and use of decision support tools approved by GNCHC to systematically assess and address patient needs.
2. Utilizes appropriate time management skills to ensure timely completion of tasks, nurse visits and other duties.
3. Assess, plan, direct and evaluate total nursing care as determined by the patient's age, related physical, psychosocial, and cultural needs in accordance with established standards, policies and procedures of the health center.
4. Provides basic education information to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.
5. Acts appropriately on nursing standing orders per established clinic policies and procedures in regards to medication refills, prior authorizations, treatments etc.
6. Utilize critical thinking skills to assess, collect and analyze patient information, prioritize and intervene in multiple encounters in daily clinic operations.
7. Administers medications skillfully and correctly, and performs lab/diagnostic testing as ordered by provider and reports all critical lab values immediately.
8. Demonstrates an ability to remain calm, perform effectively and professionally during peak periods of activity and emergency situations.
9. Utilizes universal precautions in obtaining specimens (venipuncture, cultures, etc.) and in delivery of patient care. Performs lab testing (i.e. urine dip, pregnancy test, strep screen, hema cue) including quality controls.
10. Completes and maintain clear and concise documentation in the EHR.
11. Assures a system of nursing triage that will act as a resource to processes to assess patient health care needs in relationship to access for appointment scheduling, acuity determination, and/or denial for non-emergent needs when appropriate.
12. Serves as a resource person to other staff in the area of one's own clinical expertise.
13. Works collaboratively with the nurse supervisor regarding general clinic workflow and notifies the nursing supervisor of urgent concerns immediately.
14. May assist with clinic workflow to insure smooth daily operations. This includes monitoring work in progress, assisting with flow of patients, monitoring phone flow, order completion and patient care. May aid as needed to overcome temporary workflow congestion.
15. Provides direction and education to unlicensed staff as needed.
16. As requested and needed assists with inventory management, and ordering of medical supplies and pharmaceuticals.
17. Attends and participates in required meetings, actively participates as part of the GNCHC team, builds and enhances a positive work environment.
18. Performs other jobs as assigned.
EDUCATION:
Degree from an accredited nursing program is required.
CERTIFICATIONS:
· Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes.
· Current, unrestricted State of Nebraska Nursing License.
· Current BLS certification.
REQUIRED SKILLS:
· Proficient with Microsoft Office Suite or related software.
· Ability to work well with individuals of diverse socioeconomic, cultural, and intellectual backgrounds.
PREFERRED SKILLS:
· Ability to speak and understand Spanish
