What are the responsibilities and job description for the Hospice Nurse Practitioner position at Good Samaritan Hospice?
Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. POSITION SUMMARY
The Certified Registered Nurse Practitioner (CRNP) is a licensed health care provider who collaborates with the hospice medical directors to diagnose and manage care of hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team. Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital).
QUALIFICATIONS
1. Conducts face-to-face physical examinations and comprehensive assessments of patients to determine appropriateness and eligibility for continued hospice services (recertifications)
2. Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician and hospice team
3. Works closely with the hospice medical director and associate medical directors to ensure appropriate diagnoses and treatment of hospice patients
4. Educates employees, patients, families/caregivers, facilities on hospice care and government regulations

QUALIFICATIONS
- Current CRNP license in the state of Pennsylvania
- 2-3 years experience in patient care required
- Demonstrates excellent clinical skills
- Current CPR certification
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
- Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
