• Provide skilled nursing services in the home environment utilizing a holistic person-centered approach
• Administer medications according to the plan of care
• Ensure accurate charting and documentation is completed on a daily basis
• Direct, supervise and teach assigned personnel according to the plan of care
• Work with families and other health care professionals to ensure a continuity of care
• Demonstrate the ability to make appropriate nursing judgments
• Maintain up-to-date knowledge of all fire, safety and weather procedures
• Document accurate start / stop times, mileage, incident reports and other documentation delegated by Good Shepherd Policies and ProceduresLicensed Practical Nurse / LPN - Home Care Benefits

• Must be Practical Nurse currently licensed in the state of Minnesota
• Must have a current Basic Life Support (CPR and AED) certification
• Must demonstrate leadership skills necessary to perform the responsibilities of the position
• Prefer at least one (1) year LPN experience in home care or equivalent experience
• Exhibit professional behavior and attitudes
• Demonstrates the ability to make appropriate professional judgments
• Possess good interpersonal communication skills
