The Hospice Social Worker provides medical social services to clients and their families, working as a member of the interdisciplinary hospice team.
PRN Status – Participate in on-call SW coverage for hospice admissions or routine visits as needed, one weekend per month. Complete hospice admission information and assessments as availability allows.

Responsibilities
The Medical Social Worker is responsible for:
- Assessing Significant Social and Emotional Factors for Clients and Families,
- Helping Client and Family to Understand, Accept and Follow Care Plan,
- Using All Available Means to Help Enhance Client’s Quality of Life,
- Communicating with Interdisciplinary Team Members to Provide a Holistic Approach to the Hospice Care Plan,
- Participating in Ongoing Educational Opportunities to Enhance Knowledge of Hospice Social Service Interventions.
Requirements
- Masters Degree in Social Work from an accredited university
- LSWA, LMSW or LCSW License in Kansas preferred
