General Purpose: In a few brief sentences summarize the primary duties and responsibilities.
Manage a caseload by providing individual assistance and guidance to individuals with disabilities with the mission of assisting them with obtaining, training and maintaining employment. The most prevalent disability group is Intellectual/Developmental Disability; others may include mental illness, visual impairment, mental illness, or other conditions and impairments.
Essential Functions: Describe the intrinsic job duties that define the core responsibilities of the job.
- Maintain departmental billing efficiency standards
- Maintain accurate daily case-notes, monthly summaries, SAP’s and other necessary documentation in a timely manner, on agencies Electronic Health Record Platform (EHR)
- Site visits with participants and employers throughout NYC Region in order to provide support services
- Responsible for maintaining and updating key information in EHR records to ensure a state of audit ready records
- Assist transitioning youth from school an unemployed individuals to through a formal “Discovery” to explore possible careers
- Work effectively with consumers in order to obtain/maintain employment
- Maintain charts in a timely manner and audit ready condition
- Write clear and concise case notes and service plans in a timely manner.
- Work in a cooperative manner with other Goodwill and non-GW programs.
- Effective Job Coaches will be able to develop good relationships with consumers, parents, therapists, group homes and employers, in order to facilitate positive movement toward employment goals.
- Through those relationships help consumers, to remove obstacles and barriers toward employment goals.
- Work effectively with employers in order to facilitate accommodations and address workplace problems/issues to result in greater job retention.
- Needs to maintain contact with consumers and employers to identify and resolve problems early on in order to prevent a job loss or termination from the program
- Good interpersonal skills
- Flexible Schedule and Travel throughout 5 boroughs, most time is spent out in the field at multiple sites and multiple boroughs each day
- Assists consumers in achieving the highest level of independent functioning on the job
- Coaches are encouraged to directly assist in job development activities and efforts
- Additional duties as requested by Manager
Qualifications/Basic Job Requirements: Describe the skills, knowledge and abilities required. Include minimum educational and experience requirements and preferences.
- Minimum preferred: Associates degree.
- Preferred: 1 year of experience working with people with disabilities
- Exceptions will be made for graduates of specialists DSP training programs such as Educational Opportunity Center graduates (EOC’s)
Scope of Responsibility & Positions Supervised: Where applicable describes level of authority, decision making discretion, consequences of actions, supervisory and fiscal responsibility. Provide metrics where possible. Level of supervision received.

- Job primarily consists of field on-site and off site face to face visits with consumers.
- Travel throughout the NYC mostly on public transportation
- Expected to make site visits during consumer work schedules and will consist of evening and weekend work in the community.
