Our care management division supports patient-centered care through care coordination, complex care management and helping address health-related social needs.
Job Responsibilities:
$220,000-$300,000 per year.

Job Responsibilities:
- Diagnosing and treating common childhood ailments and minor injuries.
- Recommending patients for further treatments and to specialists when needed.
- Providing preventative care, including the recommendation of vaccines.
- Recording and maintaining the health and development information of their patients.
- Effectively communicating their patients' health, nutritional, and fitness needs to their guardians.
- Medical Degree (M.D.) or Doctor of Osteopathic (D.O.)
- Graduate of an accredited residency program in family medicine, internal medicine/pediatrics, or pediatrics
- Minimum 3-5 years related experience as a practicing clinician, or the equivalent combination of education and experience.
- Must be board certified at hire and will be expected to maintain appropriate board certification, licensure, and membership in professional organizations.
$220,000-$300,000 per year.
