Position: Certified Home Health Aide (CHHA)
Location: Mountain Lakes, West Milford, Montville, Hackettstown
Full-Time Shifts
Monday-Friday | 7:00 AM - 3:30 PM
Part-Time / Per Diem Shifts
Monday-Friday | 3:00 PM - 7:00 PM or 4:00 PM - 8:00 PM
Saturday-Sunday | 7:00 AM - 3:30 PM, 3:00 PM - 7:00 PM, or 4:00 PM - 8:00 PM

What You'll Do:
As a Certified Home Health Aide at Grace Healthcare Services, every day is an opportunity to make a difference. You'll provide:
- Personal Care: Assist patients with bathing, grooming, dressing, skincare, and mobility support.
- Meal Preparation: Prepare simple, nutritious meals according to care plans and dietary guidelines
- Home Management: Maintain clean, safe, and comfortable patient living spaces.
- Documentation: Accurately log care activities using CellTrak to ensure clear communication with the clinical team
- Team Collaboration: Partner with RN Case Managers to observe, report, and respond to changes in patient conditions promptly.
What You Need:
- Active NJ Certified Home Health Aide (CHHA) license in good standing
- Communication Skills: Ability to read, write, and fluently communicate in English.
- Physical Readiness: Capable of lifting, pushing, and pulling up to 50 lbs.
- Flexibility: Dependable transportation and willingness to travel throughout Morris County
Reliable transportation is required.
