JOB SUMMARY:
Provides respite, advocacy, training of developmental life assets, and mentoring services to children/adolescents in Hillsborough County within the home and school environment. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor.

WORK ENVIRONMENT:
- High-paced, collaborative team performance with an emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate-controlled office environments with frequent exposure to clients. Requires traveling within Hillsborough County via personal vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.
FUNCTIONAL RESPONSIBILITIES INCLUDE:
- Provide therapeutic mentoring/respite services in a timely and reliable manner in accordance with the policies, procedures, and applicable provisional standards.
- Complete all documentation based on established timelines as per program requirements.
- Provides responsible activities with consistent supervision to clients and their families.
- Keep the direct supervisor and treatment team informed at all times of relevant consumer and family issues.
- Maintain efficient workflow management and organization as per program requirements.
- Meets with case manager and family to establish specific details for implementing home and community-based rehabilitative and respite services.
- Other duties as assigned or needed as per program requirements
QUALIFICATIONS:
- Associate's degree in Human Services related field or high school graduate with one (1) year of experience with adolescents in a clinical or recreational setting with Children.
- Must be, at minimum, 21 years old with a valid Florida Drivers License. Ability to read, analyze, and coordinate with multidisciplinary team regarding procedures, and governmental regulations.
- Ability to write reports and document mentoring services and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.
