DEFINITION:

As a Dietitian, you will be responsible for assessing clients' nutritional needs, developing and implementing personalized nutrition plans, and providing ongoing support and education. This role is crucial in promoting overall health and wellness by addressing dietary concerns and collaborating with the client’s team to promote wellness. This position reports directly to the Integrated Health Services Director.

EDUCATION AND EXPERIENCE:

Requires a bachelor’s degree in nutrition, Dietetics, or a related field. A Registered Dietitian (RD) credential or eligibility for RD certification and state licensure or certification as required by applicable regulations is required.

KNOWLEDGE AND SKILLS:

  • Knowledge of nutrition principles and guidelines.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong knowledge of nutrition science, dietary guidelines, and therapeutic nutrition.
  • Excellent communication and interpersonal skills for effective patient interaction.
  • Ability to work collaboratively with a multidisciplinary healthcare team.
  • Compassion and empathy in addressing patients' nutritional needs and concerns.
  • Proficiency in using nutrition assessment tools and electronic health record systems.
  • The willingness and ability to become Wellness Certified.
  • An expectation of core competencies in relation to individuals with co-occurring disorders.

JOB DUTIES AND RESPONSIBILITIES:

This position is responsible and held accountable for the following duties:

  • Understands and promotes our Certified Community Behavioral Health Clinic (CCBHC) concept by providing or supporting community-based mental and substance use disorder services; advancing integration of behavioral health with physical health care; assimilating and utilizing evidence-based practices on a more consistent basis; and promoting improved access to high quality care.
  • This job does require the selected candidate to possess a current and valid driver’s license, to have a clean driving record and reliable transportation.
  • Employee must be willing and able to perform all job-related travel normally associated with this position (this may include transportation of clients).
  • Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
  • Meet or exceed “satisfactory” expectations at any scheduled job performance evaluation.
  • Regular and predictable attendance is essential and expected for this position.
  • Conduct nutritional assessments to evaluate clients' dietary habits, medical history, and specific health conditions.
  • Develop individualized nutrition plans based on assessment findings, considering clients' dietary preferences and restrictions.
  • Collaborate with healthcare teams to integrate nutrition plans into overall client care, especially for individuals with chronic conditions or undergoing medical treatments.
  • Provide one-on-one counseling to clients’, offering guidance on healthy eating habits, portion control, and lifestyle modifications.
  • Monitor clients' progress and adjust nutrition plans as needed, taking into account changes in health status or treatment protocols.
  • Educate clients and their families on nutrition-related topics, including the importance of dietary choices in managing and preventing health issues.
  • Stay informed about the latest research, trends, and developments in nutrition science to provide evidence-based recommendations.
  • Conduct group education sessions and workshops on nutrition and healthy living.
  • Maintain accurate and thorough documentation of patient assessments, interventions, and progress notes.
  • Stay compliant with relevant regulations and ethical standards in the practice of dietetics.
  • Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
  • Participates in staff meetings.
  • Must meet “satisfactory” expectations at any scheduled job performance evaluation. Quality of work will be evaluated through training, skill, or special purpose. Must be competent and adequate.
