SUMMARY
The Social Worker/Anishinaabek Family Services (AFS) Adult Case Worker provides case management counseling, direct service and court advocacy to victims of family violence, and provides direct service to tribal member accessing financial support services. A key focus of this position involves improving access to health services, providing individual assistance, information, and advocacy for the Elders. Furthermore, this position will work to increase the education, outreach, and enrollment of Elders in a variety of federal, state, local, tribal, social service, and Veterans programs.
MINIMUM QUALIFICATIONS
- Must have a Bachelor's Degree in Social Work.
- A minimum of two years of experience working with the elderly and their families.
- At least one year of experience in a social services field.
- Must have a valid unrestricted driver license and be insurable by the GTB insurance carrier.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Coordinate with Elders, nutrition, health, and other social service programs to provide outreach to Elders on benefits for which they are eligible.
- Make presentations at organizational events, in particular Elder's functions.
- Prepare, coordinate, and submit informational articles to organizational newsletters.
- Maintain and provide assistance, particularly for the Elders, in completing enrollment forms for various federal and state social service programs
- Monitor and receive referrals from all tribal programs, tribal Elders, and provide case management services.
- Determine client needs, and develop a case plan
- Complete documentation to include weekly itineraries and daily logs
- Assist in the completion of monthly, quarterly and year-end budget reports and narrative reports
- Conduct office and home visits, interact with clients, and provide transportation to service agencies
- Facilitate weekly support group for victims of family violence
- Assist in annual grant writing
- Attend meetings held by the court and other victim service providers
- Visit area domestic violence shelters and agencies as needed
- Maintain human services and family violence prevention program client files
- Assist the Program Director in the responsibilities of grant and contract administration
- Provide education to the community and other GTB Departments on Elder abuse, advanced planning on care giving.
- Assist in the investigation of Elder abuse allegations and collaborate with police, courts and other agencies to resolve problems.
- Coordinate with State Dept. of Human Service on Elder abuse and concerns.
- Serve as patients advocate by working with courts, hospitals, nursing homes, foster homes, police and other Tribal services.
- Develop and maintain clinical records in accordance with tribal, state and federal regulations.
- Participate in weekly Anishinaabek staff meetings and Health Department meetings as scheduled and other meetings as requested by your supervisor.
- Pursue and attend trainings as required or necessary, i.e. veteran's benefits, health, social services, retirement programs, etc. to keep skills updated.
- Maintain patient confidentiality as per Health Insurance Portability and Accountability Act (HIPAA) of 1996 and complete HIPAA compliance training.

- Recognizes technical problems or deviations from normal procedure, and confers with immediate supervisor to resolve them after searching precedents to develop a recommended solution.
- Uses good judgment in interpreting guidelines, in adapting department procedures, or in recommending approaches or solutions for specific problems.
- Knowledge of the special developmental and psychological needs of the elderly is essential.
- Familiarity and ability to work with the Native American community is necessary.
EDUCATION and/or EXPERIENCE
- Bachelor's Degree in Social Work required. A Master's Degree in Social work is preferred.
- A minimum of two years of experience working with the elderly and their families is required.
- Current Social Work License through the Department of Licensing and Regulatory Affairs Office or ability to obtain licensure within the first year of employment.
- 1 year experience in a social services field required; 2 years of experience preferred
- Requires adherence to the Ethical Standards of this profession.
DRIVING REQUIREMENTS
- Must have a valid unrestricted driver license and be insurable by the GTB insurance carrier.
- Will be required to provide a current copy of your MVR (Motor Vehicle Report).

TYPICAL PHYSICAL DEMANDS
