GrandView Health at Home is hiring for part-time PRN RNs and LVNs to provide in home care through both our home health and hospice at home lines of care. Home health consists of visits in the home setting in coordination with other disciplines, while hospice consists of the same home style visits, in a more palliative capacity. There is plenty of opportunity for earning potential! Through both business lines, with flexible scheduling, easy documentation through our web-based platforms, and the ability to work remotely.

RN/LVN Primary Function:

We are looking for compassionate RNs and LVNs for immediate hire to provide assistance in personal care services in patients homes in accordance with an established care plan.

Responsibilities

  • Provides nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plan.
  • Functions as a team member demonstrating collaboration with, and responsibility to patient, physician and interdisciplinary group.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses pain and notifies the registered nurse when pain is not manageable on the current medications.
  • Responds to the physical, mental, social and emotional needs of chronically ill patients and their families.
  • Performs patient care responsibilities considering needs to the standard of care for patient’s age.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Addresses and supports cultural practices, as long as such practices do not harm others.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Supports and maintains a culture of safety and quality.
  • Adheres to all Home Health, payroll, billing and documentation policies and procedures.
  • Participates in Home Health’s other mandated activities, as requested by the Clinical Nurse Manager including inservices and UR/PI committee.
  • Flexible, yet organized, with ability to exercise sound judgment.
  • Understands Home Health philosophy and issues of death and dying.

Skills

  • Proven working experience as RN/LVN/LPN
  • Ability to monitor vital signs and to collect specimens
  • Familiarity with basic nutrition and personal hygiene standards
  • Familiarity with infection control and current protocols i
  • Bilingual encouraged, not required
  • Nursing and health care administration skills
  • Caring and compassionate personality
  • Current CPR certificate
  • First Aid training
  • Valid driver’s licence
  • Current nursing license
  • Experience with Kinnser software a big plus

Requirements:

Education: Currently licensed by the Texas Board

Experience: Minimum of one year in a home health or hospice care setting. Two years experience in Home Health preferred.

  • Able to maintain strict confidentiality with regard to protected and sensitive information.
  • Able to establish and maintain confidence of owners and employees.
  • Must be able to solve problems independently and confidently by applying analytical and logical thinking.
  • Able to use standard office equipment and software proficiently.
  • Strong team player

  • Self-Starter and does well with Autonomy.

Language Skills:

  • Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations.
  • Able to write reports and correspondence at a professional level.
  • Able to immediately respond to common inquiries and complaints from employees and regulatory agencies.
  • Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
  • Able to effectively organize and present information and respond to questions from employees and external groups of varying sizes.

Job Type: Part-time

Pay: From $35 per hour for RN, from $25 per hour for LVN
