Job Description: Adult Case Manager
Title: Adult Case Manager (Sanford, ME)
Reports to: Case Management Program Manager
Salary: Starts at $18.50/hour (dependent on experience)
Hours: Full-Time, Hybrid Schedule
Summary:
Case Managers are part of a team who provide Adult Disability Services Case Management services to individuals with intellectual & developmental disabilities and Autism. The position is 40 hours/week with access to paid time off and health insurance. Case Managers report to the Case Management Program Manager and receive support from the Clinical Supervisor as needed.
Essential Duties and Responsibilities:
· Advocate for and assist with applying for MaineCare services including medical, housing, employment, transportation, community-based services, Section 21 and 29 DS Waiver funded services and other services as needed.
· Meet regularly with members to ensure progress, health and safety, quality of life, and address any concerns in these areas.
· Assist with applying for, and maintaining Social Security benefits, EBT, Medicaid, and financial management.
· Coordinate and monitor medical, dental, vision and psychiatric appointments.
· Maintain routine contact with members, their support team and collateral contacts for ongoing assessment of member needs and safety.
· Coordinate, review, and document person-centered plans, care plans and behavior plans.
· Maintain accurate case records of assessment, activities, and plans in the electronic database.
· Follow and maintain reporting requirements.
· Ensure records are submitted within required timeframes.
· Discuss and document progress, challenges, and barriers to success with the individual and the treatment team.
· Evaluate impact and efficiency of services.
· Monitor community-based Medicaid Waiver programs and document progress and outcomes.
· Meet minimum required billable hours on a quarterly basis.
· Participate in crisis resolution using crisis intervention techniques and skills as well as collaborate with Adult Protective Services and Crisis Prevention/Intervention Services when necessary, in order to stabilize and secure emergency needs.
· Other duties as assigned for the benefit of the program.
Qualifications:
· Holds a bachelor’s degree from an accredited four (4) year institution with a specialization in psychology, behavioral health, social work, human services, special education, counseling, rehabilitation, nursing, or a closely related field.
· Must have at least 3 years of experience in the field of social services.
· Must have a valid driver’s license, a violation-free driving record, and use of a personal vehicle.
· Demonstrates a solid ability to manage time and tasks independently and successfully.
· Learns and applies knowledge of the complex State of Maine DHHS regulations and guidelines, along with the timelines associated with support and service needs.
· Possesses strong skills in positive and effective verbal and written communication, excellent organizational skills, conflict resolution, problem solving, time management, independent in task completion, and decision-making skills. LSW or LSX preferred.

· All candidates will be subject to driving and criminal background checks. A valid driver’s license, current vehicle registration, inspection, and insurance are also required at time of hire.
