Key Responsibilities
• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement integrated discharge plans
• Provide crisis intervention and emotional support

• Offer patient and family education on healthcare services and resources
• Supervise graduate social work student interns
• Perform mandated reporting duties as required
• Facilitate appropriate referrals to community services

Job Requirements
• Master's degree in Social Work from an accredited program
• LMSW or LCSW licensure in New York State (or eligibility)
• Minimum 2 years of healthcare-related experience

• Strong crisis intervention skills
• Excellent interpersonal and communication abilities
• Proficient computer skills, including EMR systems
• SBIRT (Screening, Brief Intervention, and Referral to Treatment) certification (or willingness to obtain within 6 months)
